Ocular Fundus Changes in Toxemia of Pregnancy

The toxemias of pregnancy are one of the great triad of complication responsible for the vast majority of maternal deaths. Ophthalmologists are frequently dealt with the diseases because of their visual and vascular involvement. There were many authors who reported the characteristic pictures of ocular fundus findings found in toxemia patients, but still few reported their relationship to other general features. Ocular fundi of 305 cases of toxemia patients were examined, we obtained the following results. 1. The ocular fundus changes in toxemia of pregnancy mainly consist of hypertensive retinopathy and also show generalized edema of retina. The appearance of edema is like that of seeing the bottom of bucket through water. 2. The possiblity to develop hypertensive retinopathy is much higher and much severer in eclampsia than in pre-eclampsia. 3. When the diastolic pressure is below 90 mm Hg, the presence of hypertensive retinopathy is more frequent in eclampsia. 4. When either one of proteinuria or general edema is accompanied by hypertensive retinopathy, the condition may be assumed as severe. According to the above mentioned results, the ocular fundus should be examined for every pregnant patient, especially when the obstetrician faces to make decision of termination of pregnancy.
