經由Pterion施行眼窩腫瘤摘除之臨床經驗

Clinical Experiences in Pterional Approach to the Orbit for Removal of Orbital Tumor

摘要


眼窩腫瘤摘除術,除常用的Krönlein-Berke術外,對於眼窩外側後方或肌錐內之腫瘤或蝶骨瘠之腦膜瘤之摘除,採用“經由pterion切開眼窩術”有許多優點。 此手術乃沿髮線切開,而於pterion(額骨、頂骨、顳骨䚬部、蝶骨大翼之交叉點)附近鑽鑿骨孔,進入眼窩壁而將腫瘤摘除。因不需切開腦膜故減少腦內併發症之危險,其所鑽骨孔之部位及大小可隨腫瘤位置而改變,對特殊部位之眼窩腫瘤摘除較易施行,而髮線切開可得較佳之美容結果。此手術較Krönlein-Berke術費時且對顳肌之傷害較大是其缺點。 本文提出二十四例手術之臨床經驗。

The technique of pterional approach is described for opening the orbit by way of a single trephine through the Pterion (the junction of the frontal and parietal bone, the temporal squama, and the greater wing of the sphenoid). This approach is especially indicated for removal of orbital tumors of posterior lateral orbit, muscle cone and those of sphenoid ridge meningioma. Among 24 operated cases, 17 cases were performed by ophthalmologist alone, while the first 7 cases were combined with neurosurgeon. Steps of operation were described. The results and complications were discussed. The hair line incision permits better cosmetic result. This approach permits a more extensive removal of roof and lateral posterior orbital wall than the transfrontal operation or Krönlein-Berke operation. Although there are some advantages of this operation, it takes more time and get more injury to the temporalis muscle and small vessels as compared with Krönlein-Berke procedure.
