高度近視眼之白內障囊內及囊外摘除手術之比較

摘要

收集自民國71年至73年三年間，接受白內障摘除手術的高度近視193眼，佔全部老年性白內障摘除手術的8.42%。平均近視度數約兩千度。依摘除手術的不同，分成囊內及囊外兩組。手術中的併發症有玻璃體脫出、後囊破裂及麻醉時穿剌眼球等。經過平均23.5個月的追踪後，對115眼進行詳細的眼前葉及眼底檢查，發現12%有視網膜裂孔。囊外組幾乎都有後囊環帶狀的纖維化，故平均術後視力不如囊內組。術後視網膜剝離的發生率為5.3%，主要出現在囊內組。本篇討論手術後的併發症與手術中的併發症之間可能的關連性，並比較兩種白內障摘除手術的安全性。

We collected 193 cataract extractions in eyes with high myopia (over 6 diopters) in our hospital between 1982 and 1984. The mean of phakic refraction was about -20 diopters. We examined the peripheral fundus of 115 eyes thoroughly by indirect ophthalmoscope and Goldman three mirror during the last follow up visit (the mean follow up period was 23.5 months). They were divided into two series, one consisting of 73 intracapsular cataract extractions and the other consisting of 42 extracapsular cataract extractions. The surgical complications, including vitreous prolapse, capsule rupture and penetration of globe during retrobulbar anesthesia, were described. The incidence of silent retinal break was high (12%) in both series. The incidence of aphakic retinal detachment was greater in the intracapsular series (6.9%) than in the extracapsular series (2.4%). Ring-shaped posterior capsule opacity was found in nearly every case after extracapsular cataract extraction. The relation between the surgical complications and the post-operative retinal complications, and also the differences in results between intracapsular and extracapsular cataract extractions were discussed.
