摘要

本篇報告五例高度近視患者接受角膜表層屈光矯治移植術的臨床結果。自民國75年12月至77年5月，我們矯治了4位病人（共5眼），其中一位兩眼皆接受手術。病人的年齡平均二十八歲，男性一人，女性三人，追蹤期間為40至74週，平均55.8週。手術前平均屈光度為－15.1 D，術後平均矯治13.80 D，預期矯治效果達94.09%。手術20週以後，兩例有近視回複(regression)的現象，術前皆屬嚴重之高度近視患者。追踪40週以上，五例角膜片皆移植成功，裸眼視力皆較術前為佳，沒有不良之後遺症發生。對於不想戴眼鏡或隱形眼鏡的高度近視患者，Epikeratophakia是一種可逆、安全又有效的屈光手術。

We report the clinical results of epikeratophakia for the correction of high myopia with Kerato-Lens in five eyes of 4 patients. There were one male and 3 females. The follow-up period ranged from forty to 74 weeks, with an average of 55.8 weeks. The average difference from emmetropia was 15.1 diopters before surgery (range 10-22 D), and -1.3 diopters after surgery (range -4.75 D undercorrection, +1.0 D overcorrection). The five eyes achieved an average of 94.09% of the desired correction. We have two patients who have experienced myopic shifts in their refraction from 5 months postoperatively. All the grafts were successfully implanted and all the patients improved uncorrected visual acuity. For high myopic patients who are spectacles or contact lenses intolerant, epikeratophakia can provide a reversible, safe and effective form of refractive surgery.
