網膜芽細胞瘤之冷凍治療－病例報告
Cryothreapy for Retinoblastoma-A Case Report
摘要
網膜芽細胞瘤是一種好發在小孩眼部之惡性腫瘤，臨床上以白瞳孔、斜視（尤其是內斜視）、失明及續發性青光眼為其特徵。其治療方式甚多，包括眼球摘除、放射線治療、化學治療、光凝結、熱治療及冷凍治療。一般而言，眼球摘除是優先考慮的治療，然而對於一隻眼睛已因網膜芽細胞瘤摘除，另一隻眼睛亦發現網膜芽細胞瘤，為保留患者之視力，其治療方式則有待斟酌，而冷凍治療為其變通方式之一。

A 3-month-old baby was sent to our clinic on June 6, 1984 with the chief problem of white pupil of left eye for one month. Under indirect ophthalmoscopy of the fundus, a well-defined whitish subretinal nodule about 1.5 D.D. over temporal upper quadrant of right eye and total retinal detachment with a yellowish subretinal mass of left eye were found. Under the impression of retinoblastoma, the left eye was enucleated. The pathology study comfirmed the diagnosis. Cryotherapy with the triple freeze-thaw cycle for the right eye was done for three times later. The tumor was forzen at -80℃ for 30-60 seconds with CO2 cryo-probe. The applications were sustained until ice was seen to envelop the tumor. This procedure was repeated in a freeze-thaw cycle three times. Postoperatively, the tumor mass disappeared with only chorioretinal atrophy over the treated area without macula involvement. No recurence was noted till now, and the child enjoyed his life without further aid.
