Retinal Detachment in Wagner's Vitreoretinal Degeneration-Report of a Family

Wagner's氏玻璃體網膜退化症所引起之網膜剝離－家族病例報告

摘要

本文報告一家族成員中的兄妹二人，具Wagner's氏玻璃體網膜退化症之特徵；光學上中空之玻璃體，周邊網膜具有許多小的裂孔並有玻璃體牽引，白內障，近視，以及再發性之網膜剝離。病例一在初診時右眼視力為0.05，左眼僅具光覺。右眼周圍網膜具有許多小的裂孔，我們在這些裂孔周圍打了鐳射。二年之後右眼發生網膜剝離，我們施行了鞏膜扣壓術，但是開刀之後3個月網膜再度剝離，所以再做了經平坦部玻璃體切除術，水晶體切除術，及Silicone oil的內固定。做完上述手術後，網膜的下顳側自然發生一個很大的裂孔。最近的追踪治療，病人網膜除黃斑部外都已剝離，視力為0.05。
病例2是病例1的妹妹。初診時右眼可以矯正到0.1左眼僅具光覺。右眼上方網膜剝離，我們做了鞏膜扣壓術。三個月後，顳側部分有一些新的裂孔，我們加做了一段的鞏膜扣壓，及打了一個較廣範圍的鐳射。二年多之後，右眼網膜上方再發生剝離，於是再為她做了玻璃體切除術。在最近的追踪檢查，右眼視力為眼前30公分處可分辨手指。
早期診斷出Wagner's病症是重要的，因為不管是預防的治療或是發生網膜剝離時的處理都相當困難。在這二個病例，我們以玻璃體切除術及Silicone oil的內固定，結果並不理想，這種不良的開刀結果似乎肇因於這個網膜本身的特質。

Two members of a family with the characteristics of Wagner's vitreoretinal degeneration were reported. The polymorphous ocular signs of these patients, including optically empty vitreous, multiple retinal breaks with stringy vitreous strands inserted in peripheral retina, cataract, myopia, extensive area of white without pressure and recalcitrant recurrent retinal detachment. Early diagnosis of this disesase is important either for prophylactic or therapeutic surgery of the detached retina. We describe herein the natural course and the experience of surgical treament of retinal detachment in Wagner's disease.
