Mooren's Ulcer Treated with Lamellar Keratoplasty
目倫氏潰瘍以半層角膜移植
摘要
兩位雙側性目倫氏潰瘍的病人，以傳統眼部及全身性的藥物治療無效，且潰瘍持續進行至角膜穿孔。這兩位病人接受半層角膜移植手術和類固醇的治療後，進行性潰瘍得到控制，同時眼球構造和功能得以保存。半層角膜移植術之所以對目倫氏潰瘍有效，可能是藉著去除角膜抗原的刺激，並形成一道屏障避免發炎細胞的侵入和免疫反應。此種手術對即將或已角膜穿孔的目倫氏潰瘍病例爲一有效之治療方法。

Two patients with progressive bilateral Mooren's ulcer were unresponsive to conventinal ocular and systemic therapy. Even prednisolone, colchicine and cyclophosphanside (endoxan) were used in one case. Lastly, both of them received lamellar keratoplasty and steroid treatnsent. The progressively destructive inflammatory process was arrested, and ocular anatomy and function were preserved in both two patients during the one year of follow-up. The procedure may work by the removal of a corneal antigenic stimulus as well as the tormation of a barrier preventing the invasion of inflammatory cells and immune reaction. Lamellar keratoplasty can be used in the impending and actual perforation cases of Mooren's ulcer.
