球後麻醉後的視力變化

The Effect of Retrobulbar Anesthesia on Visual Acuity

摘要


球後麻醉在眼部手術中,仍是一種主要的麻醉方法。它能限制眼球運動,以及有效的阻斷痛覺傳導,因此有利於手術的進行。向於球後麻醉是將麻醉劑注射到眼肌錐內,對視神經也會產生麻醉的效果,而此則少為人提及。本篇報告30位(30眼)接受白內障手術的病人,手術前施行球後麻醉之後視力降低的情形。有56.7%(17眼)視力降至30公分辨指數以下,其中有6.7%(2眼)無光覺,16.7%(5眼)僅存光覺。而視力降低的程度與眼球運動受限制的程度有平行關係。另外從光投射的變化,證實有視野缺陷產生。

Retrobulbar anesthesia remains one of the most common methods of anesthesia used in ophthalmic surgery, as it can achieve akinesia and anesthesia of the globe and facilitate the surgeons task. Since the anesthetic is injected into the muscle cone, it does appear to affect the optic nerve. Nevertheless, few clinical studies have been done to determine the effect. We examined 30 patients for visual acuity, ocular motility, and light projection before and after retrobulbar anesthesia for cataract surgery. Twenty minutes after the anesthetic was injected, 17 (56.7%) of these patients had the vision of counting finger or worse. Of these patients, 2 (6.7%) had no light perception and 5 (16.7%) had light perception only. The change of light projection also reveals that there were visual field defects after retrobulbar anesthesia.
