視神經盤黑素細胞瘤－三病例報告

摘要

三位病人（年齡分別為62，42，28歲）視神經盤上之深色腫瘤均在偶然機會下發現，它們的大小由半個至兩個視神經盤不等，甚至整個蓋住視神經盤，此三例均位於視網膜表層且有不規則之邊緣，其中最大一例已有往視網膜內散播，螢光攝影檢查顯示並無供應之血管且後期無螢光劑滲漏，超音波檢查亦顯示腫瘤內部呈強反射，廣泛的視野缺損（只剩餘中央5度）可在最大例中發現，而另兩例之視野則幾乎正常，最大例之電學檢查顯示已有異常，而這顯示神經已受壓迫，我們認為這三例都是黑素細胞瘤，但是仍須非常謹慎小心追踪，特別是有無惡性變化之可能，這些都是我們必須謹記在心的。

Three patients (aged 28, 42, and 62 respectively) were found to have hyperpigmented lesions during routine ocular examination. The size of the lesions varied from half to double disc size. The lesions were all located in the superficial layer of the retina with fibrillated margins and intraretinal spreading in the largest one. In two cases, fluorescein angiography showed no intralesional feeding vessels and no specific intralesional late leakage. Ultrasonography all revealed high internal reflectivity as solid mass. Extensive visual field defect (5 degree remained) occurred in the largest one but otherwise normal field in the small and medium ones. The Pattern VEP of the largest case showed delayed latency and reduced amplitude, which indicated that nerve fiber layer compression has already occurred. We thought that these were all disc melanocytoma but guarded regular follow- up and the possibility of malignant change and intravitreous spreading, especially in the largest one, should be kept in mind.
