角膜移植病患之斜視矯正－病例報告
Strabismus Correction after Penetrating Keratoplasty-Case Report
摘要
對於角膜白斑併發斜視的病人，常需角膜移植和斜視手術來恢復視力和外觀，兩項處理的先後順序皆具挑戰性。基於角膜移植傷口所需癒合時間較長，而斜視手術易引起屈光變化等考量，通常選擇先行斜視矯正，再做角膜移植；但角膜移植的不確定因素常導致斜視復發，而前功盡棄。在此提出兩例先行角膜移植再做斜視矯正的臨床經驗，並加以分析和討論：我們認為眼球經角膜移植恢復視力後進行斜視手術，其效果除可矯正眼位，且兩眼同視功能也可獲得恢復，是一項值得參考的處理方式。

The management of leukoma cornea combined with strabismus and the priority of penetrating keratoplasty or strabismus surgery are challenging. We successfully performed recession and resection on 2 patients with exotropia 14 and 25 months after penetrating keratoplasty. The result showed that not only ocular alignment but also binocularity can be restored by the strabismus surgery on the recovered eyes which were deprived by the acquired disease. We suggested recession and resection is the second procedure after penetrating keratoplasty in patient with acquired leukoma cornea combining strabismus.
