美國辛辛那提高齡角膜移植之結果

Results of Penetrating Keratoplasty Using the Aged Donor Corneas from Cincinnati, U.S.A.

摘要

本文報告高醫眼科在1989年7月到1990年8月之間，使用了67顆美國辛辛那提眼庫提供的高齡角膜施行角膜移植。其追蹤時日為3個月到15個月（平均8個月），角膜捐贈者平均年齡大於70歲。本文按照手術目的及豫後分類分別探討其手術結果如下：治療性角膜移植12例中有50%成功率；光學性角膜移植共55例，其中預估豫後良好組14例其成功率92.8%，預估豫後尚佳組29例，其成功率達72.4%，在預估豫後不良組12例，其成功率達33.3%，本研究結果顯示這些高齡角膜移植的結果與年輕角膜相差無幾。這給我們一個啓示：高齡角膜在角膜供應短缺之現在仍可考慮作為捐贈角膜之一個來源。

Sixty-seven high aged donor corneas from Cincinnati Eye Bank (USA) were used for penetrating keratoplasty during the periods of July, 1989 to August 1990 in Kaohsiung Medical College Hospital. The average follow-up of 8 months (3 months to 15 months) were studied according to the surgical aim and prognostic criterias for the visual outcome and corneal clarity. The results revealed that there was 50% successful rate in the therapeutic group (12 cases) and variable outcome in the optic aim group (55 cases): 92.8% (14 cases) of clear rate in the favorable prognostic group, 72.4% (29 cases) of clear rate in the fair group and 33.3% (12 cases) of clear rate in the poor group. The result of this study is not much different from that using younger donor corneas. It suggests that as the shortage of donor corneas being a problem to us, the high aged Cincinnati donor corneas could be a choice of corneal source for penetrating keratoplasty in Taiwan.
