晶體前表面弧度與閉鎖性青光眼的關係

The Correlation between Anterior Lens Curvature and Primary Angle Closure Glaucoma

本研究收集了130個眼睛,將其分成四組:年長正常組,PACG (Primary Angle Closure Glaucoma)組,年長POAG (Primary Open Angle Glaucoma)組,年青POAG組。嘗試以影像的數位分析(EAS-1000),測量其晶體前表面弧度、前房隅角、前房深度和瞳孔大小。結果發現:PACG組的晶體前弧南率半徑(8.8mm)顯著的比其他三組小(10.5~11.4mm)。另外,迴歸分析發現;年齡與晶體前孤曲率半徑(r=-0.413)、瞳孔犬小(r=-0.638)、前房深度(r=-0.538)和前房隅角(r=-0.635)均為負相關。前房深度與晶體前弧曲率半徑為正相關(r=+0.268)。我們認為,隨著年齡增加,晶體前表面孤度將愈陡(steep),前房深度會愈淺,瞳孔也會愈小,這些變化將使人患PACG with pupil block機會增加。

Introduction: Under slit lamp, we found the curvature of anterior lens surface seemed to be steeper in patients with PACG. In order to confirm our suspicion, we studied the relationship between anterior lens radius and glaucoma. Method: In our clinics, we collected 130 eyes which were then divided into 4 groups: elderly normal group, PACG group, elderly POAG group and young POAG group. The digital analysis of the image of the anterior eye segment was used to measure the radius of anterior lens surface, anterior chamber depth, anterior chamber angle and pupil size. Result: The radius of anterior lens surface in PACG group (8.8mm) was significantly smaller than that in the other 3 groups(10.5-11.4mm). The correlation between age and the parameters including anterior lens radius, AC depth, AC angle and pupil size were negative relationship. A positive correlation between anterior lens radius and AC depth was found. Conclusion: The smaller radius of anterior lens surface is an important risk factor for PACG because the iris will be easier to adhere to the anterior lens surface.
