中心視野與視力之相關性

The Correlation between Central Visual Field and Visual Acuity

摘要


目的:研究青光眼病患,當其中心視野喪失時,視力將受到何種影響。 方法:由青光眼病患中,篩選Humphrey 30-2視野檢查出現中心視野部份缺損之患者,經排除患有白內障或視網膜病變之眼睛後,再以Humphrey 10-2檢查其中心視野,然後檢定患者Humphrey 10-2及30-2視野測量中最中心四個測量點之敏感度喪失(Sensitivity loss)與患者最佳矯正視力的相關性。 結果:總共22位男性、7位女性,共39隻眼睛。年齡為28至80歲(平均67.4歲)。青光眼類型分別為:17隻原發性隅角開放型青光眼(Primary open angle glaucoma)、14隻低眼壓型青光眼(Low tension glaucoma)、7隻慢性隅角閉鎖型青光眼(Chronic angle closure glaucoma)、l隻ICE症(Iridocorneal endothelial syndrome)併發續發性青光眼(Secondary glancoma)。相關性分析顯示:在Humphrey 10-2視野檢查中最中心四個測量點,如有兩點以上(含)的敏感度喪失達15dB以上時,該眼視力將明顯下降;在Humphrey 30-2視野檢查中,則無此相關性存在。在全部39隻眼睛的最中心四個視野測量點(總共156點)中,30-2視野檢查中有91點敏感度喪失達15dB以上,而重複以Humphrey 10-2視野測試後,僅剩63個點有如此嚴重的缺失。 結論:Humphrey 10-2視野檢查比Humphrey 30-2更能準確的評估患者中心視野的狀態,當Humphrey 10-2視野檢查最中心的四個測量點,如有兩點(含)以上其敏感度喪失大於15dB(含),該眼視力將明顯下降。

Purpose: to study the relationship between central visual field loss and visual acuity in glaucomatous patients. Methods: A total of 39 eyes in 29 glaucomatous patients were analyzed retrospectively for the relationship between Snellen visual acuity and central visual field. The central visual field was determined by Humphrey 10-2 and 30-2 programs. Results: there were 22 male and 7 female. Age ranged from 28 to 80 years (average 67.4 years). The types of the glaucoma were: 17 primary open angle glaucoma, 14 low tension glaucoma, 7 chronic angle closure glaucoma, and 1 ICE syndrome with seconddary glaucoma. Correlation analysis showed that a sensitivity loss of over 15dB in at least 2 of the most inner four points in 10-2 program would cause a significant decrease in visual acuity. In 30-2 program, no such correlation was found. In 30-2 program, among the 148 most inner points (37 eyes), 91 points had over 15dB sensitivity loss while in 10-2 program only 63 points had such defect. Conclusion: Humphrey 10-2 program was more accurate in determining the status of central vision. A sensitivity loss of over 15 dB in more than two of the most inner four points will cause significant decrease of visual acuity.
