黃斑部視網膜前出血之釹雅克雷射玻璃體膜切開術治療：三病例報告

摘要

目的：報告三位黃斑部視網膜前出血之病患，在利用釹雅克雷射玻璃體膜切開術來進行治療後，使黃斑部前出血迅速被消除吸收，達到快速恢復視力之情況。
方法：自2004年起，在本院門診病患中收集黃斑部前出血之病例，其中第1例為增殖型糖尿病性視網膜病變患者、第2例為老年性黃斑病變，患者、第3例為車禍造成外傷性黃斑部病變患者。利用釹雅克雷射技術穿破後部玻璃體膜造成開洞，而使黃斑部視網膜前出血迅速向玻璃體腔內擴散，促使出血早期被吸收或消失，以迅速恢復視力之治療方法。
結果：在利用釹雅克雷射玻璃體膜切開術治療之後，立即可見黃斑部前出血迅速向玻璃體腔內擴散，三病例之視力在雷射治療之後有立即之改善，而在10天之後所量測之最佳矯正視力均獲得快速之恢復，矯正視力之進步如下：病例1由0.08至1.0，病例2由0.07至0.2，病例3由0.1至1.0。長期追蹤之下並未發現任何已知相關視網膜或脈絡膜傷害、出血或視力下降等情形。
結論：以釹雅克雷射玻璃體膜切開術，對黃斑部視網膜前出血患者施術應可獲得良好之視力預後及達到早期治療之目的，並提供黃斑部前出血另一種比傳統玻璃體切除手術更低侵入性的治療選擇。

Purpose: We report the visual outcome of Nd: YAG laser hyaloidotomy in three premacular hemorrhage patients.
Method: Three cases arc reported. Each patient was selected from different etiologies; proliferative diabetic retinopathy, age-related macular degeneration and traumatic maculopathy. Nd: YAG laser hyaloidotomy was performed to create an opening in posterior hyaloid membrane. Premacular hemorrhage was released into vitreous cavity for faster absorption.
Results: Visual acuity was restored in approximately 10 days after Nd: YAG laser hyaloidotomy, from 0.08 to 1.0, 0.07 to 0.2 and 0.1 to 1.0 in each patient, respectively. Follow-up examination showed no signs of retinal or choroidal injury.
Conclusion: We conclude that Nd: YAG laser hyaloidotomy may be an effective procedure for selected premacular hemorrhage, which is a less invasive alternative compared with conventional vitrectomy.
