摘要

目的：報告一Scedosporium apiospermum角膜炎的病例。
方法：病例報告及相關文獻回顧。
結果：一位41歲男性在左眼被污水濺傷後產生紅眼，淚眼及眼睛疼痛等症狀，角膜潰瘍培養出Scedosporium apiospermum。在以每小時點一次的natamycin眼藥水治療後臨床症狀出現部分改善，之後再以淺層角膜刮除術及每兩小時一次的amphotericin B治療後，角膜表皮細胞缺損及基質發炎細胞浸潤逐漸改善，追蹤至出院後3個月的最佳矯正視力為0.4。
結論：早期及正確的診斷和藥物治療並適時的手術可以讓Scedosporium apiospermum角膜炎的病人有較佳的癒後及視力。

Purpose: To report a case of Scedosporium apiospermum keratitis.
Method: A case report and literature review
Results: A 41-year-old male presented with redness, tearing and pain of left eye after a minor injury. Microbiologic examination of the corneal scrapings revealed Scedosporium apiospermum. Partial clinical improvement was achieved with hourly topical natamycin. Superficial keratectomy was then performed and topical amphotericin B was added every 2 hours. The epithelial defect and stromal infiltration gradually resolved. The best-corrected visual acuity was 20/50 three months after discharge.
Conclusion: Accurate detection and prompt antifungal treatment with adjuvant surgical intervention may preserve ocular integrity and visual acuity in patients with Scedosporium apiospermum keratitis
