Over a 2 month period all seriously-ill patients or their companion arriving at Keelung Chang Gung Memorial Hospital emergency department were asked to complete a questionnaire. We wished to identify those factors which affected their decision as to whether or not to utilize the EMS. A total of 200 persons were questioned. Only 38% made use of the 119 ambulance. Over 60% considered speed and convenience the first priority when transproting a sick patient to hospital. The provision of prehospital care was considered of secondary importance. Sex, age, educational level, and relationship to patient did not make a difference in their perception. After explaining that the 119 ambulance was operated by trained EMTs able to provide basic prehospital care and a rapid response, about 90% of the respondents agreed they would use the EMS in the future. While the importance of rapid transport for seriously ill patients appears well understood by the public, they were largely unaware that the Keelung EMS was both rapid and able to provide prehospital care leading to underutilization. With minimal education they appeared willing to change their practise in the future.
