護理人員下背痛及其職業相關危險因子之調查
Low Back Pain and Occupationally Related Risk Factors among Nurses
摘要
下背痛是一個骨骼肌肉方面常見的問題，在歐洲及美國下背痛已是臨床護士最常見的健康問題之一“本研究的主要目的在探討國內護理人員下背痛的盛行情況，並希望藉此研究找出與護理人員下背痛相關的危險因子，作為防治護理人員下背痛之參考。本研究採橫斷性研究設計，以自填式問卷對中部某區域教學醫院及南部某市立醫院的所有護理人員進行調查，調查的時間為民國八十四年二月，總樣本為 546人。研究結果顯示，國內護理人員罹患下背痛的情形相當嚴重，過去一年內的盛行率約 77. 7 % ， 有 21 .2 ％的護理人員曾因下背痛而服藥， 19.2 ％因此去看醫生。彎腰與站立時間與下背痛有關，工作中彎腰及站立時間愈長罹患下背痛的情形愈高。此外，各項護理活動中以幫病人翻身、拍背、復健、擦澡與下背痛的相關性較高，每週每做 10 小時這項護理工作其過去七天內下背痛的 POR ( Prevalence Odds Ratio)為 2 .33 ( 95 % CI 1.09- 4 .99 ) ，其次為量體溫血壓 POR 為 1.77 ( 95% CI 1.07 -2.93)，搬病人、點滴、器材等 POR 為 1.39 ( 95 % CI0.98-1.97)。至於這些護理工作中哪些因素是造成下背痛的主因，則仍需進一步有關人因工程（er - gonomics）的研究。

Low back pain is the most common musculoskeletal disorders. In European countries and the United States , low back pain ( LBP ) has been reported to be one of the most common health problem among clinical nurses. The main purpose of this study is to assess the prevalence of LBP in nurses and to explore the relationship between potential occupational risk factors and LBP . A cross-sectional survey was conducted in a total of 546 nurses from two teaching hospitals. Self-administered questionnaire was used to collect personal information and disease histories. The result of this study showed the prevalence of LBP in recent one year was 77 . 7 %. Moreover , 21 . 2 % of nurses have ever taken medicine and 19.2 % have reported seeing a physician for relief of LBP. Long duration of stooping and standing was associated with LBP. Some special nursing tasks also showed to increase the prevalence of LBP. The POR for helping patients bedside rehabilitation , chest care and bathing in bed was 2.33 ( 95 % CI 1.09~4.99 ). The POR was 1.77 ( 95 % CI 1.07~2.93 ) for taking temperature or blood pressure. Assisting patients get into or out of wheelchair and bed or carrying equipment have an POR of 1.39 ( 95 % CI 0.98 ~1.97 ) . The results imply that a good design of work postures and nursing activities is needed to prevent LBP in clinical nurses.
