數位乳房攝影巡迴車乳癌篩檢追蹤分析研究：以台灣中部某區域醫院為例

Follow-up analysis of breast screening by mobile breast screening car: experience of a regional hospital in central Taiwan

摘要

自從數位乳房攝影巡迴車啟用投入乳癌篩檢的業務之後，每年皆有穩定且大量的篩檢個案數，為中部地區提供可近性高的乳房篩檢服務。從數位乳房攝影巡迴車乳癌篩檢追蹤分析研究中可發現，數位乳房攝影巡迴車的陽性個案比率稍高，但是回診比率與接受組織生檢的比例卻偏低，另外其乳癌篩檢真實陽性率也偏高，顯示數位乳房攝影巡迴車的篩檢個案樣本也許被污染，在後續的治療及追蹤也與院內的篩檢個案情況略有不同。本篇將針對數位乳房攝影巡迴車乳癌篩檢追蹤作討論，分析其背後的可能原因與其對應措施。

Since the mobile breast screening project participate the breast screening service in our hospital, there are large amount of cases every year, providing a convenient way to receive the breast screening in central Taiwan. On the basis of the follow-up analysis, we found the recall rate is slightly higher, but much of the cases do not visit the clinic and the ratio of biopsy rate is also lower in the positive mobile breast screening group, this is somewhat different as compared with the population who receive the screening mammography in hospital. Besides, the true positive rate of breast cancer in this population is relatively high, possibly due to ＂sample contamination＂ because that some symptomatic breast cancer patients may have used the free breast screening service. In this article, we will discuss the results of the follow-up analysis of breast screening by mobile breast screening car, to find the underlying causes and the way to resolve them.
