Medial Plantar Flaps for the Reconstruction of Hand and Foot Defects

摘要

內側足掌皮瓣對於手掌與腳掌之缺損重建，是一個非常理想的組織供給者，因為這些部位需要較為特別之皮膚覆蓋。在過去一年多來，我們收集了一共六個病人，接受了內側足掌皮瓣的移植。其中有三例，我們使用內側足掌游離皮瓣，來重建病人手掌之缺損；另外三例，則使用內側足掌島狀皮瓣來重建病人足部缺損。這六個病人術後的追蹤為五至十六個月，除了一位病人因有部分植皮於皮瓣供應區脫落須再次植皮外，都顯示令人滿意的結果。皮瓣供應區的合併症有麻木感、角質增生、和色素沉澱。內側足掌皮瓣提供足夠的體積，與四周組織的顏色和質感相當吻合。對於重建手掌與腳掌，是一個非常好的選擇。

The medial plantar flap is an ideal tissue reserve for the reconstruction of the plantar and palmar defects. It provides the unique form of the skin. In the past one year, we treated six patients with the medial plantar flaps for the reconstruction of hand and foot defects. Free medial plantar fasciocutaneous flaps were employed for reconstruction of palmar defects in three cases. Island pedicled flaps were employed for reconstruction of soft tissue defects on feet in three cases. The follow-up period ranged from 5 to 16 months. The overall results were satisfactory except partial skin graft loss at flap donor site required re-grafting in one case. The donor site morbidity includes numbness, hyperkeratosis, and pigmentation. The medial plantar flap has sufficient bulkiness and a well skin color and texture match of the surrounding tissue. The medial plantar flap is an excellent choice in the reconstruction of palmar and plantar areas.
