應用健康信念模式探討慢性阻塞性肺部疾病患者運動實施之相關因素

摘要

本研究目的在應用健康信念模式探討影響慢性阻塞性肺部疾病患者運動實施之相關因素。採用問卷調查法，使用「人口學資料之問卷」、「個人運動特徵問卷」及「運動之健康信念量表」箏研究工具進行訪談，在南部某醫學中心及區域醫院，以立意取樣方式共收集60位慢性阻塞性肺疾病門診患者。資料分析包括描述性統計、卡方檢定分析、獨立t檢定、皮爾森積差相關、對數複迴歸分析(multiple logistic regression)。結果發現影響慢性阻塞性肺疾病患者是否實施運動之重要因素爲「醫師是否告知病患要運動」與「運動障礙」，其實施運動之勝算比分別爲未規律運動者的1.66倍(OR=1.66; 95% C.I.=1.05~2.21)與2.22倍(OR=2.22; 95% C.I.=0.24~0.84)。本研究結果顯示健康信念模式中的「障礙」因素、與醫師是否告知病患要運動的「行動線索」對慢性阻塞性肺疾病患者運動之實施影響很大，因此，醫護人員應仔細評估慢性阻塞性肺部疾病運動實施之障礙，作爲設計活動之參考，並應運用各種場合向病患強調運動的重要性。

The purpose of this study was to use the Health Belief Model (HEM) to analyze the factors related to the exercise practice of Chronic Obstructive Pulmonary Disease (COPD) patients. Purposive sampling was used to recruit sixty COPD subjects from the outpatient department of two hospitals in southern Taiwan. Data were collected using a structured questionnaire developed by authors. Demographic data, characteristics of personal exercise, and belief model of exercise were measured. Data were analyzed using descriptive statistics, Chi-square test, t-test, Pearson correlation, and multiple logistic regression. The results were as follows. ”Told to exercise by doctor” (the ”cue”) and ”perceived barriers” were the main factors affecting the exercise practice. Respectively, they were 1.66 times (OR=1.66, 95% C.I.=1.05~2.21) and 2.22 times (OR=2.22; 95% C.I.=0.24~0.84) higher than for patients who did not exercise regularly. Two HBM factors-the ”cue” (told by physician) and ”perceived barriers” - may influence COPD patients' exercise practices. Therefore, physicians and nurses need to carefully evaluate the ”cue” and ”obstacles” of exercise practice when designing activities and emphasizing the importance of exercise for COPD patients.
