五年制護生初次內外科護理學臨床實習之經驗

The Lived Experiences of Student Nurses during First Stage Medical-Surgical Nursing Clinical Practice

摘要


本研究屬質性研究,旨在以五專護生的觀點,深入暸解其於初次內外科護理學實習的歷程感受及如何因應。採立意取樣法,共收集12位北部某技術學院五專三年級護生的資料。於護生實習結束後一週內透過一次約60~90分鐘的深度訪談收集資料。訪談內容予以錄音,再轉寫成行爲過程記錄,並以內容分析法進行語言資料的分析。結果顯示,護生初次內外科護理學實習的歷程感受可歸納爲:對將面臨之實習情境的不可預測感、面臨身心再受重整的壓力、受困於學習壓力的煎熬、陷於自尊受傷害的窘境、實習過程與預期不相吻合、如釋重負並體認新的自我。其衍生的因應行爲,可歸納爲:嘗試確認並掌握將面臨的臨床情境、竭力增進自身專業能力、努力裝備及維護自我、自我強化拼命力撐、勇於嘗試專業學習與成長、回顧統合實習經驗。研究結果可供日後輔導護生初次內外科臨床實習時之參考。

The purposes of this qualitative study are to describe the lived experiences of junior student nurses and understand their coping behaviors during the first stage of Medical-Surgical nursing clinical practice. The researcher selected twelve subjects who attended first stage Medical-Surgical nursing practice by purposive sampling. Data were collected by conducting one-hour in-depth interview, one week after clinical practice. The verbal and non-verbal behaviors expressed by the students during interview were then audiotaped and recorded in a narrative process-recording. By means of content analysis, the verbal behaviors of subjects were systematically analyzed and categorized. The results show that the lived experiences of the students can be categorized as follows: sense of uncertainty about clinical practice situation, threat to the sense of physical-psycho reconstruction, trapped within suffering due to learning stress, falling into predicament as to the hurt of self-esteem, sense of mis-match about clinical practice situations, sense of accomplishment and becoming a new self. The coping behaviors expressed by these students can be categorized as follows: trying to confirm and manage their own clinical practice situation, striving to improve the professional competence, engaging in equipping and maintaining self, sticking it out by self-empowerment, being bold in learning and growing to maturity from nursing profession, reviewing and integrating of clinical practice experiences into self as the nursing students. The result of this study can be used as references for guiding students during first stage of medical- surgical nursing clinical practice.
