摘要

本文描述一位61 歲男性個案，因為梗塞型腦中風導致活動功能障礙，面對疾病恢復進展緩慢及對未來失去信心之護理經驗。照護期間自2013年9月5日至9月15日，以觀察、傾聽、會談、身體評估、查閱病歷及出院電話訪問方式收集資料，並運用Gordon十一項功能性健康評估指引，確立個案有：無力感、健康尋求行為、身體活動功能障礙之護理問題，筆者藉由關懷及陪伴等方式與個案及主要照顧者建立良好護病關係及信任感，提供個別性的照護，瞭解個案的需求及困難，共同擬定適合個案之健康維護活動計劃，並給予專業及適切性的知識，協助個案正視本身之無力感，增強復健動機並參與治療，改善肢體活動功能，達到增強力量與信心。

This case report describes the nursing experience of caring for a 61-year-old male patient with impaired physical mobility after stroke. From September, 15 to September, 29 in 2013, the author assessed the patient with Gordon 11 Functional Health Status Assessment tool, collectin relevant data through observation, conversation, listening and physical assessments. According to the medical records and phone interview, the patient's problems were powerlessness, health-seeking behavior, and impaired physical mobility. During the process of caring, a good relationship and trust were built. Empowerment Theory was utilized by the author for the guidance in the nursing process to provide individual care and to encourage expression of feelings. The needs for rehabilitation activities was arranged individually. By applying professional and appropriate nursing care, the author helped the patient face his feelings, improved his confidence to overcome obstacles and powerlessness, so that patient could find out the meaning and value of life at the end of the process.
