This article described the nursing experiences of assisting a stroke patient and her caregiver from March 19th to April 16th 2018. The authors utilized Gordon's 11 functional health scales for collecting information, and to analyze and sort out the health problems based on the patient's physical assessment, close observation and interview with the patient and her significant caregiver. The major problems were: "Mobility impaired-physical", "Dysphagia" and "Caregiver role strain". The authors spent time with the patient and her caregiver together to better establish a good client-nurse relationship. Then, active care, concern, and empathic listening was provided to relieve the caregiver's stress and to encourage him to describe difficulties and any problems to caring for the patient. Relevant nursing activities were provided to offer the caregiver know-how for improving his ability to take care the patient. Interdisciplinary care models were also integrated to help the patient to overcome physical disability and difficulty swallowing with a regular rehabilitation program including swallow training, prevention of limb contractures and occurrence of aspiration pneumonia, with the aim of improving quality of life for the patient and her caregiver.
