Improvement of the Accuracy of Indwelling Urinary Catheter Care in Foreign Caregivers

摘要

2017年1－10月居家留置導尿管個案泌尿道感染率為6.5%，進一步瞭解泌尿道感染的22位個案中，有16位（72.7%）為外籍看護照顧，故引發專案動機，發現外籍看護照顧正確率僅58.5%，原因有溝通困難、缺乏衛教工具、未查核導尿管照顧情形；為提升外籍看護執行導尿管照顧正確率，經製作雙語導尿管衛教手冊、QR Code影片教學、雙語五要一不客製化提醒板、建立家屬查核機制等措施，於2018年12月導尿管照顧正確率提升至94.1%，2018年8－12月降低居家個案泌尿道感染率為1.4%，運用團隊合作結合家屬共同參與照顧，提升外籍看護技能，能有效改善照顧品質。

From January to October in 2017, the rate of urinary tract infections in in-home care cases with urinary catheter was 6.5%. Further investigation indicated that 16 of the 22 cases (72.7%) with urinary tract infection were undergoing care by foreign caregivers. Additionally, as a consequence of the difficulty in communication, insufficiency of suitable tools for care instruction, and lack of supervision on urinary catheter care, the accuracy of urinary catheter care performed by foreign caregivers is only 58.5%. The object of this project is to improve the accuracy of urinary catheter care performed by foreign caregivers. By making manuals and reminder posters for care instructions, QR Code for video guides and establishing a check system for family members, we raised the accuracy to 94.1% in December of 2018, and meanwhile reduced the rate of urinary tract infections to 1.4%. We conclude that the quality of in-home care can be improved sharply by collaborative participation by family members of the patient, homecare nurses, and foreign caregivers with enhanced in-home cares abilities.
