摘要


在牙周病治療當中,引導組織再生術佔有相當重要的地位,各種再生膜之使用技術雖然已發展成熟,但仍有其限制。如何使引導組織再生術效果更能夠預期,手術更易施行,在其材料方面仍有發展空間,因而有牙釉質衍生物(enamel matrix derivatives, EMD)之出現,本篇即是針對此材料之臨床使用,綜合數篇近年來的研究結果,加以整理比較之文獻回顧與探討,做為牙周病治療時臨床應用的參考。

The finial goal of periodontal therapy consists not only of arresting the tissue destruction caused by periodontal disease, but also of reconstructing periodontal tissue and maintaining their health over the years. Currently guided tissue regeneration (GTR) with or without membrane play an important role in periodontal regeneration therapy. However, GTR techniques still present some unsolved problems: (1) a limit in the amount of regeneration achievable because of membrane position; (2) membrane contamination in cases of exposure; (3) delayed wound healing because of degradation of reabsorbable membranes; and (4) use of membranes requires a good degree of surgical skill and long healing period. Ala solution to these problems, the use of growth and cell differentiation factors has been proposed. Enamel matrix derivative (EMD) is available as a commercial formulation in Taiwan, and has recently been introduced as a new modality in regenerative periodontal treatment. This article has been based on the limited studies published so far as a literature review for the use of EMD in periodontal intrabony defects.
