非埋入式植牙合併引導骨再生術：病例報告

埋入式兩階段植牙（two stage, submerged implant）手術時，若發生骨缺損時可以合併引導骨再生術，只要再生膜不過早露出，則效果是可以預期的；而非埋入式一階段植牙（two stage, nonsubmerged implant）發生同樣骨缺損的狀況時，是否可以採取類似於埋入式兩階段植牙合併引導骨再生術則仍未定論。本病例報告患者為48歲女性，右下第二小臼齒、第一大臼齒及左下第一、二小臼齒及第一大臼齒區缺牙，計劃以非埋入式一階段植牙作口腔復建。左右兩邊進行植牙時都發生冠向處骨量缺損不足的現象，因此取自體骨和三鈣磷酸鹽（tricalcium phosphate）混合充填骨缺損處，並覆以可吸收的膠原蛋白再生膜（collagen membrane）類似自然牙齒施行引導組織再生手術方式。術後傷口癒合平順，並嚴格控制口腔衛生。4個月後完成贗復製作。全部復建後成果良好，囊袋深度均小於3mm且根尖X光片顯示骨頭狀況良好，因此提出報告並探討此種手術方式的可預期性。

Guided bone regeneration has been used for the two-stage submerged implant for many years. The clinical result is predictable if the barrier membrane is not early exposed. The same concept is also applied on the one-stage nonsubmerged implant placement. Recently, absorbable membrane is used for this technique since the absorbable time of membrane is much extended. The report presents a case with five one-stage nonsubmerged implants which were combined with bone graft and absorbable barrier membrane to solve the inadequate bone volume on the coronal third region. Prosthesis was finished four months later. Clinical results from this case are favorable. The detailed procedures are related literature are also discussed.
