Treatment of Dens Evaginatus by Regenerative Endodontics and Indirect Pulp Capping: Case Report

以再生性根管治療及間接蓋髓術處置牙中阜:病例報告

摘要


牙中阜常見於亞洲人種。這些牙齒的凸出物容易會有外傷或被摩耗。其中百分之七十的牙中阜內含牙髓組織,而且常在萌發之後很快這些牙髓組織就被暴露出來。未完全發育成熟的牙齒在感染後可以以再生性根管治療處理,但其實一個簡單的治療來預防後續的併發症。本篇報告的患者因為中間阜感染導致蜂窩性組織炎來院求診,我們使用氫氧化鈣來降低牙髓內的感染,但是情況卻未被控制下來。為預防其他感染的情況,其他有中間阜的牙齒都先以預防性間接蓋髓術處置。長期的結果顯示牙根都有持續的發育,顯示此項處理方式可以是一個有效的預防治療。

Dens evaginatus is a developmental dental anomaly occurs predominantly in Asian descent. These tubercles are prone to wear down or fracture. 70% of dens evaginatus contains pulp tissue which may be exposed soon after eruption. Periapical abscess, cellulites resulting from dens evaginatus are common emergent complication which can be treated with regenerative endodontics in immature teeth. These conditions can be prevented by a simple prophylactic approach. The patient in this report suffered from cellulitis originated from infection of dens evaginatus. Calcium hydroxide was used to dressing the infected tooth, but the infection was not controlled. We do preventive indirect pulp capping for the other teeth with dens evaginatus. The long term prognosis revealed this prophylactic treatment can preclude further infection and the roots would continue to grow.
