牙髓壞死恆齒以revascalarization治療方法誘導牙根再生之病例報告

摘要


對於牙根發育未完全且牙髓壞死的牙齒,根管治療真是一大挑戰。以往的研究多用氫氧化鈣來進行根尖成形術,這樣的治療需要長時間來誘導牙根底部形成硬組織屏障。長期放置氫氧化鈣會改變牙本質的機械性質,通常會有較薄且脆弱的牙根,或是牙冠-牙根比例不佳,往往容易發生牙根斷裂。牙根發育未完成的牙齒,其牙髓組織具有豐富的血管供應,及修復受傷組織再新生的能力。當有效地移除受感染且壞死的牙冠牙髓組織,並達到適當地消毒殺菌,保留根尖的活細胞,以利再生新牙髓組織來填滿根管。本文為病例報告,針對牙根未成熟的小臼齒,伴隨根尖周圍炎或膿包,治療時盡量不破壞根尖組織,並以大量2.5%次氯酸鈉沖洗,在根管的上三分之一放置氫氧化鈣。病患於術後半年,放射線影像下可見其牙根繼續生長及根管壁變厚,且無其他後遺症。臨床上對於牙髓壞死且牙根未成熟的牙齒使用revascularization technique來取代傳統的根尖形成術,可提供組織再生,牙根繼續形成,而得到一較佳的結果。

The purpose of this report was to present two clinical cases of immature permanent premolars that developed periradicular periodontitis or abscess underwent revascularization technique. e disinfection of the canal space was involved with copious 2.5% NaOCl irrigation and calcium hydroxide dressing at coronal third of root canal. These cases presented continued root formation without complications, although narrowing canal space and calcified barrier were observed. The mechanism of this continued development and formation of the root end are discussed. Furthermore, this treatment approach can help rescue infected immature teeth by physiologically strengthening the root walls.
