摘要

本病例為一下顎前突之骨性三級患者，沒有顯著的水平覆蓋差異；治療上以正顎手術合併旋轉咬合平面的方式取代傳統去代償的觀念與作法。從結果來看，這種治療方式的好處為：矯正治療簡單化、縮短整體治療的時程、減少拔牙、同時改善微笑曲線。關於在正顎手術中以旋轉咬合平面的方式來改變上、下顎骨相對關係的作法，其運用的概念、設計方式、長期的穩定性、可能的副作用、以及臨床應用等也會在本篇文章中作討論。

An alternative surgical treatment plan was presented to show how to efficiently correct mandibular prognathism without large overjet discrepancy presented. The protrusive chin was eliminated through clockwise rotation of the occlusal plane and maxillo-mandibular complex. Within this method, no complicated orthodontic treatment was needed and the total treatment time was therefore shortened. The smiling arc was also improved. The philosophy, clinical application, long term stability, and side effects of the occlusal plane manipulation in orthognathic surgery were discussed.
