A Project to Improve the Rate of Accuracy of Aerosol Therapy in Pediatric Wards

摘要

本專案的目的是提升兒科病房噴霧治療執行正確率，於查房時發現噴霧治療執行不佳，調查原因為：1、家屬對執行目的及使用方式不清楚；2、病童害怕蒸氣的聲音及煙霧；3、更換照護者頻繁，家屬間未交班；4、護理人員對噴霧治療認知不足。於2017/4/3～10/31擬定改善措施，舉辦在職教育、修訂噴霧治療標準作業、制定衛教單張及拍攝衛教影片、設計治療性遊戲教案等，結果顯示照顧者在接受噴霧治療之後執行正確率由12.3%提升至93.1%；家屬間交班比率由13.4%提升至86.7%；護理人員對噴霧治療的認知由52.4分提升至100分，本專案可提供兒科病房照護之參考。

The purpose of this project was to improve the rate of accuracy of aerosol therapy in pediatric ward. Inadequate implementation of aerosol therapy in a pediatric ward was found during ward rounds. Possible causes were: (1) caregivers were unclear about the purpose and methods of the therapy; (2) the hospitalized children were afraid of the noise and fumes made during therapy; (3) frequently changes of caregivers without handover between family members; (4) inadequate understanding on aerosol therapy by the nurses. From April 3 to October 31, 2017, we established improvement measures, including continuing education, revision of the SOP of aerosol therapy, preparation of education flyers and films, and design of therapy games. Results showed that the rate of accuracy of aerosol therapy increased from 12.3% to 93.1%. The cooperation rate of the hospitalized children increased from 25.4% to 98.3%. The rate of handover by the caregivers rose from 13.4% to 86.7%. The nurses' knowledge of aerosol therapy rose from 52.4 points to 100 points. This project serve as a reference for other forms of pediatric care.
