摘要

背景：隨著平均壽命的延長，失智症是老人常見之慢性疾病。因其常伴隨有精神行為症狀，如何減緩家庭照顧者的負荷已成為重要的議題。目的：比較於記憶或一般門診接受治療之失智症患者之家屬照顧負荷的差異，以及影響家屬照顧負荷的因素。方法：採橫斷式相關性研究，以方便取樣100位在北部某醫院失智症患者家庭照顧者，使用量度照顧者負擔問卷及自擬門診跨團隊服務量表來比較接受記憶門診或一般門診照護（精神科或神經內科）其家庭照顧者感受之負荷差異。結果：相較於接受一般門診照護者，記憶門診組的家庭照顧者總負荷明顯較低。針對失智症提供的跨團隊服務中，病情告知、臨床症狀及心理調適與家庭照顧者總負荷呈顯著負相關，表示提供病情告知、臨床症狀處理及心理調適服務越多，則家庭照顧者感受總負荷越低。結論：研究發現記憶門診提供之跨團隊服務內涵對降低家庭照顧者負荷是有幫助的，建議可將此跨團隊服務內涵推廣至各醫療院所。

Background: As life expectancy increases, dementia is a common and chronic condition among the elderly. Because behavioral and psychological problems are highly prevalent among dementia clients which will cause sufferings for their family caregivers, how to reduce the load of their burden have become an important issue. Purpose: The aim of this study was to compare the family caregivers' burden of dementia patients who received memory or general outpatient clinics treatments and its related affecting factors. Methods: The cross-sectional correlational research was used. A convenience sample of 100 family caregivers of dementia patients were surveyed using the Caregiver Burden Inventory and self-developed multidisciplinary team service scale. Subjects were recruited from the memory and psychiatric or neurological outpatient services of one hospital in Northern Taiwan. Results: The total burdens of family caregivers for patients receiving memory clinic was significant lower than those who receiving general outpatient services. Among multidisciplinary team services provided, briefing patient's condition, explaining clinical symptoms and providing psychological adjustment had significant negative correlation with the total burdens of family caregivers. As family caregivers perceived more services provided by the multidisciplinary team, the lower total burdens of family caregivers would be presented. Conclusion: The study finds that the memory clinic promotes the multidisciplinary team service which includes reducing the burden of family caregivers to be helpful. The study therefore suggests that we should promote this model of multidisciplinary team service to other health care facilities.
