膝部等速向心運動對正常年青人心肺反應之影響

Cardiopulmonary Responses to Knee Isokinetic Concentric Exercise in Young Healthy Humans

摘要


背景與目的:過去研究探討阻力運動對心血管的反應,大多在等長和等張運動下觀察,很少探討等速運動對心血管之影響及性別因子是否影響結果,因此本文主要目的在於探討膝部等速向心運動計畫對於正常年青人心肺反應之影響及性別是否影響其結果。方法:本實驗受試者共有22名(男性10名,女性12名),所有受試者先進行最大運動之實驗測試(同時記錄心跳率、血壓、攝氧量、氣體交換率等),其次再進行右腳膝部最大等速向心運動計畫為分別在角速度60°、120°、180°、240°、240°、180°、120°、60°/s下收縮五次,在每次測試速度之間休息30秒,且在每一角速度運動下量測心跳率、血壓、攝氧量、呼吸交換率等。結果:男女性別在等速向心運動過程中,心跳率、血壓、及氣體交換率未有顯著差異;在等速運動中平均收縮壓(95-157毫米汞柱)、舒張壓(56-93毫米汞柱)、心跳率(64-127次數/分鐘)、攝氧量(3.8-16毫升/公斤/分鐘)和氣體交換率(0.72-1.27)等變化皆在安全範圍。結論:此膝部等速向心運動計劃中血壓、心跳率、攝氧量及氣體交換率之變化仍屬於安全範圍。在各個角速度時之攝氧量約佔最大攝氧量之10-54%,因此運用此等速向心運動計畫於健全年青人在臨床屬有效的肌力訓練與安全適量之心肺刺激。

Background and purposes: The cardiopulmonary responses of resistance exercise during the isometric exercise and isotonic exercise were been studied in the past. However, very few studies have examined the cardiopulmonary responses to the isokinetic exercise. Thus, the purpose of this study was to investigate the cardiopulmonary responses to knee isokinetic concentric exercise in young healthy humans. Methods: Twenty-two young healthy adults were recruited for this study. All subjects first performed the maximal exercise test and during the test, heart rate (HR), blood pressure (BP), minute ventilation (VE), oxygen consumption (VO2), pulse oxygen saturation(S(subscript p)O2) and gas exchange ratio value (R) were recorded. A Kin-Com 500H dynamometer was used for the concentric isokinetic exercise of knee. The knee concentric extension and flexion exercises were performed at the speeds of 60°, 120°, 180°, 240°, 240°, 180°, 120°, and 60°/sec. A thirty-second rest was allowed between each testing speed to reduce the effect of fatigue. Results: The sex factor did not affect HR, BP, VO2, VE, S(subscript p)O2 and R during the knee concentric exercise. The SBP (95-157 mmHg), HR (64-127 beat/min), VO2 (3.8-16ml/kg/min) and R (0.72-1.27) during the knee concentric exercise were obviously within the safe range. Conclusion: The results indicated that the values of the cardiopulmonary responses (HR, BP, VO2, VE, and S(subscript p)O2) during the knee concentric isokinetic exercise were within the safe range. The oxygen consumption in every angular velocity during the knee concentric exercise was 10%-54%of VO2 max with mean of the maximal aerobic capacity testing. Therefore, the results of the study suggest that the knee concentric isokinetic exercise was the appropriate and safe rehabilitation program for strengthening in young adults.
