癌症生存者的運動指引

Exercise Guideline for Cancer Survivors

摘要


自民國71年起,癌症即高居臺灣十大死因之首,隨著早期癌症篩檢與醫療技術演進,癌症預後與存活率顯著改善。為協助癌症生存者長期對抗腫瘤疾病與癌症治療衍生的副作用,運動與癌症相關的實證醫學在近代蓬勃發展,運動被認為能有效預防癌症發生、預防慢性疾病、降低癌症與其治療副作用並提升生活品質、提升治療耐受性。美、澳等國癌症協會倡導將運動納入常規癌症照護,指出運動對於癌症照護的重要性與病患從中獲得的益處。癌症是多系統疾病,物理治療師於提供癌症生存者運動處方前必須考量患者身上可能的常駐管線、骨質健康狀態與血液檢查數值,個別化擬定運動計畫並依病情不斷修正。癌症生存者應避免靜態的生活模式,有氧、阻力性和有氧結合阻力性運動對於一般的癌症生存者是安全的,建議每週應維持合計150分鐘中強度(目標心跳介於60~79%最大心跳)或75分鐘高強度(目標心跳介於80~89%最大心跳)的身體活動。至於癌症生存者中的亞族群,像是嚴重疲勞、外科手術後、骨骼健康不佳與癌症末期等,建議轉介給物理治療師進行專業評估與運動指導。

Cancer has been the first place of the causes of death in Taiwan since 1982. With the advancement of early cancer screening and medical technology, the prognosis and survival rate of cancer have improved significantly. In order to assist cancer survivors to fight against tumors and the treatment-related adverse effects for a long time, the evidence based medicine of exercise for cancer has fl ourished recently. Exercise is believed to be effective in preventing cancer, preventing chronic diseases, reducing treatment-related adverse effects, and improving treatment tolerance and quality of life. The American Cancer Society and Clinical Oncology Society of Australia advocated the inclusion of exercise in routine cancer care, guiding the importance of exercise for cancer care and the benefits that patients derive from it. Cancer is a multi-system disease, before providing exercise prescriptions for cancer survivors, physical therapists must consider the patient's possible resident pipeline, bone health and blood test values, and formulate individual exercise plans and continuously modify them according to the condition. Cancer survivors should avoid static lifestyles, aerobic, resistance and aerobic combined resistance exercises are safe for general cancer survivors. It is recommended to maintain a total of 150 minutes of moderate-intensity (60-79% maximal heart rate) or 75 minutes of high-intensity (80-89% maximal heart rate) physical activity each week. As for the subgroups of cancer survivors, such as people with severe fatigue, post-operations, poor bone health, and terminal status, it is recommended that they should be referred to a physical therapist for professional evaluation and exercise guidance.
