Tracheal laceration or rupture is a rare but potentially life-threatening complication of endotracheal intubation. We present four female patients with tracheal laceration after emergent endotracheal intubation in the emergency department (ED) during a two-month period in 2006. The initial presentation was acute subcutaneous emphysema on physical examination, confirmed on chest radiograph. Prompt bronchoscopy was performed, and tracheal laceration was confirmed in all patients. Two of the patients underwent surgical repair without complication and tracheal rupture was proved. The other two patients received conservative management. During the hospital course, extubation was successful in three of the patients. One patient became ventilator dependent due to the underlying disease. All four patients recovered uneventfully.
