脊髓損傷患者植入人工陰莖之性復健―個案報告

摘要

脊髓損傷後的患者依然可以再次擁有性生活並享受性愛所帶來的愉悅！雖然脊髓損傷後陰莖勃起功能嚴重受損，但內心「情慾」及「性需求」仍然強烈，脊髓損傷息者年紀輕；性復健是提升生活品質的指標，幫助他們性復健可提升生活品質。
本案例描述一位第十胸椎至第一薦椎完全損傷的三十六歲男性患者；經由性諮商及泌尿專科醫師的評估與治療，使用適合個案具個別性的性復建方式，完成性功能的重建。性復建過程中經歷服用威而鋼、使用真空吸引器及陰莖緊縮環，但因陰莖硬度不足，經常無法持續維持勃起狀態，效果不穩定且無法預期。最後以手術方式植入人工陰莖，克服勃起功能障礙的問題，目前擁有滿意的性愛生活。成功的性復建，另一個重要的關鍵在於性伴侶的支持、耐心與配合。

Although spinal cord injuries impose serious damage to men's erectile ability, their desire and libido remain intact and they have the rights to enjoy sexual life, especially young people. Sexual life is an important indicator of quality of life. Sexual rehabilitation to young patients with spinal cord injury will improve their quality of life.
We reported a 36-year-old male patient, who sustained a complete spinal cord injury at the level of 10th thoracic vertebra. An individualized plan of sexual rehabilitation was accomplished. He first received sildenafil citrate and vacuum constriction device to restore his erectile ability. However, the results were not satisfactory. Finally, he received the penile prosthesis implantation and led a happy sexual life since then. The key elements of successful sexual rehabilitation are also attributed to the support, patience and cooperation from his spouse.
