建構健康醫院組織評估量表之研究

The Construction of a Measurement Scale for Healthy Hospital

摘要


目的:本研究旨在透過相關文獻的分析與探討,瞭解健康的醫院組織之構面與內涵,並透過嚴謹的量表發展程序,蒐集資料進行實證分析,藉以發展一套完整而週延,且適合台灣醫療機構的本土化評量工具。 方法:本研究首先以文獻探討法發展出健康醫院組織評估量表之基礎架構及內容,共包含「政策與領導」、「物理環境」、「社會環境」、「健康實踐」及「組織資源」等五個構面及50個衡量題項,之後經由專家就量表題項進行修訂與評估,完成量表初稿。為驗證量表初稿之適切性及信效度,本研究以立意抽樣法抽取國內地區級以上醫院員工400人進行量表施測,以蒐集資料進行實證分析。 結果:量表初稿經由信度分析、項目分析及因素分析後,共刪除七個題項。在效度的驗證上,經過二次因素分析後共萃取出四個因素,並依據題項內容重新命名因素構面名稱,分別為「健康資源」、「健康文化與活動」、「政策與領導」及「物理環境」,四個因素的解釋總變異量為67.437%,顯示有不錯的建構效度;在信度驗證上,四個因素構面之信度水率皆在0.9以上,顯示本量表具有相當高的可靠性。 結論:本量表最後共有四個構面43個題項,將各構面題項在量表中採亂數排序,並採用李克特式五點式評量尺度,分成很不同意至很同意五個選項,進而完成「健康醫院組織評估量表」。

Objectives: The purpose of this study was to develop a complete measurement scale for healthy hospital in Taiwan by conscientious and careful process of scale construction. Methods: Following a review of key literature and the use of expert opinion, a multidimensional measure of healthy hospital was developed including five dimensions as following: policy & leadership, physical environment, social environment, health practices, and organization resource. The draft scale consisted of 50 items grouped into the five dimensions. The draft scale was utilized to survey hospital employees (n=400) and data were statistically analyzed using a variety of tests to delete unsuitable items and examine reliability and validity. Results: Totaling seven items were deleted from the draft scale after reliability test, item analysis, and factor analysis. After second factor analysis, four components with eigenvalues>1 were extracted from the rotated component matrix explaining 67.473% of the total variance, it demonstrated an acceptable construct validity. Furthermore, Cronbach's alphas were above 0.9 for four components. The four components were renamed as ”health resource”, ”health cultural & activity”, ”policy & leadership”, and ”physical environment”. Conclusions: The formal scale consisted of 43 items grouped into the four dimensions. The 43 items were sorted in the scale by random. For each item, the respondents could choose from one of five responses ranging from strongly disagree to strongly agree.
