醫院高級健檢中心醫療合作外包廠商評選模式－層級分析法之應用

Developing a Selection Model for Outsourcing Suppliers of Advance Medical Examination Centers Using Analytic Hierarchy Process

摘要

目的：本研究目的在建構高級健檢中心醫療合作外包廠商最適評選模式，全民健保總額預算的實施，各醫院間紛紛積極努力開發自費項目，其中以高級健檢中心委外合作案，爲近期醫院自費收入來源重要項目。
方法：首先透過修正式德菲法彙整專家意見，找出合適的評選準則，再以層級分析法(analytic hierarchy process; AHP)來決定準則之相對權重、排序替代選方案並選出最適健檢中心外包廠商。
結果：研究結果顯示，評選高級健檢中心醫療合作外包廠商選擇因素，在主準則部分，重要性排序依序爲營運規劃能力(0.19)，醫療品質服務(0.239)，價格完整及合理性(0.172)，專業技術人力(0.114)，公司管理能力(0.056)；在次準則部分前五項分別爲財務規劃計畫(0.197)、營運前準備計畫(0.117)、營運額分配利潤(0.115)、健檢服務醫療設備(0.111)，及檢驗檢查品質準確(0.066)。
結論：整體而言，選擇高級健檢中心委外醫療團隊廠商時，最重視營運規劃能力以及醫療品質服務。而最後健檢中心委外廠商選擇部份，依序爲C廠商(0.447)、B廠商(0.299)、A廠商(0.253)，因此C廠商爲高級健檢中心委外最佳選擇。將來醫院選擇高級健檢中心委外廠商醫療團隊時，可參考本研究所建構之評選模式。本研究建構之評選模式，將協助公立醫院客觀選擇最適醫療團隊廠商；除了實證個案醫院適用外，其他和立醫院也可以作爲評選高級健檢中心委外廠商時評估的參考。

Objective: The purpose for this research is to establish modals for evaluating appropriate outsourcing candidates. Outsourcing core medical services has become a trend. After implementation of budgeting plans by Bureau of National Health Insurance, many private medical services are developed among hospitals; one of the services, to outsource medical examination centers, is a very important income source from private medical services for hospitals.
Methods: First, the opinions of experts to the modals are organized by modified Delphi technique in order to conclude the evaluation principles. Then, by analytic hierarchy process, AHP, the importance, ranking and substitutes will be decided to pick the most appropriate outsourcing supplier.
Results: AHP results allow us to determine the order of the weights. Of priority concern is planning skills (0.419), quality of medical services (0.239), completeness and rationality of price (0.172), specialty skills and personnel (0.114) and management skills and personnel (0.056).
Conclusions: Decision making based on the AHP criteria can provide a valuable reference for public hospitals that seek outsourcing partners. In addition to enabling Department of Health authorities to understand the full implications of outsourcing, the AHP-based assessment criteria can help public hospitals to understand the nature of problems that arise during outsourcing when constructing a deluxe health examination center.
