目的:肺結核為全球性傳染病,名列台灣第三類法定傳染病。個案病房為胸腔內科肺結核疾病專責病房,隨機觀察護理人員執行護理指導的內容、方式不一致,期望以有效方案介入,達到提升入住負壓隔離病房肺結核病人護理指導之完整性,做為日後照顧肺結核病人護理指導依據。方法:以北部個案醫院胸腔內科入住負壓隔離病房肺結核病人及護理人員收案對象,實地觀察、訪談、及護理指導滿意度調查等方法深入調查及擬定改善策略。結果:護理人員執行肺結核護理指導完整性由78.2%提升至97%,病人對肺結核疾病認知由54.3%提升至92.7%,病人對接受肺結核疾病衛教指導整體滿意度由73%提升至93%,達到專案目標。結論:從護理人員端進行有效的更新肺結核護理指導內容,且計畫性的衛教流程,提供完整治療資訊;病人端運用多元數位學習方式增加對疾病自主學習及管理,施行的過程可作為其他醫療機構品質改善的參考。
Objectives: Tuberculosis is a global infectious disease, and is listed as one of the Category III Notifiable Diseases in Taiwan. The ward involved in our following investigation is specialized to accommodate the patients of the pulmonology department with tuberculosis diseases. Random observation of the nursing instruction content and methods offered by the nursing staff revealed great inconsistencies. We hoped to address this discordance using an effective program to improve the completeness of these nursing instructions for the patients admitted to the negative pressure isolation rooms in this ward. Methods: By investigating, observing, interviewing the patients and nursing staff in this specialized ward for tuberculosis diseases in Northern Taiwan, and surveying satisfaction of nursing instruction, a strategy for improvement has expected to be developed and conducted. Results: The completeness of tuberculosis nursing guidance for nursing staff increased from 78.2% to 97%, the patient's awareness of tuberculosis disease increased from 54.3% to 92.7%, and the overall satisfaction of patients with tuberculosis disease health education guidance increased from 73% to 93%, reaching the project aims. Conclusions: Efficiently updating the content of tuberculosis nursing instruction given by the nursing staff, and preparing the health education process thoroughly can provide complete treatment information for the patients. These patients can also utilize multiple digital learning methods to increase autonomous learning and management of the disease. The implementation process may be considered a reference for the improvement of nursing instructions of other medical institutions.
