The Effect of Different Loading Conditions on the Motion Behavior of the Bipolar Hip Endoprostheses

不同荷重對雙極式半人工髖關節運動模式之效應

摘要


目前雙極式半人工髖關節主要臨床問題,就是理想運動模式改變,造成術後併發症。本研究目的,就是以體外實驗的方法,找出荷重對雙極式半人工髖關節的運動模式。實驗時,利用材料測試機,施力範圍從300N到3000N,量測雙極式半人工髖關節內、外軸承間之摩擦力矩。研究結果顯示,在運動初期,於不同荷重大小之下,外軸承靜摩擦力矩較內軸承靜摩擦力矩要小,而當運動持續時,外軸承的動摩擦力矩會克服內軸承的靜摩擦力矩使內軸承開始運動。從外軸承運動轉移到內軸承運動的轉動角度,隨著不同的荷重條件而有所差異,當荷重增加,轉動角度就愈大。雙極式半人工髖關節的運動模式,除了人工關節本身的設計外,還必須考量病人體重對其功能的影響,才能完整解釋其運動的模式,以真正發揮它原先的設計功能。

An in vitro experiment was done to verify the load effect to the motion behavior of the bipolar hip endoprostheses. The single load scaled from 300 N to 3000 N was used to evaluate the frictional torque and motion behavior of both bearings in this study. The frictional torque is dependent on the load. The static frictional torque of the outer bearing is less than the static frictional torque of the inner bearing in different loading conditions at initial stage. However, the dynamic frictional torque of the outer bearing would be larger than the static frictional torque of the inner bearing as the movement continued. Therefore, the relative motion behavior would happen on the outer bearing at the initial stage then shift to the inner bearing as the movement continued. In different loading conditions, the rotation angle to shift motion from outer bearing to inner bearing was different. As the load increased, the rotation angle would be larger to shift the motion. For the bipolar hip endoprostheses, surgeons should consider more factors than the indications of the patients to get better clinical results in the long run.
