社區憂鬱老人自殺評估與篩檢指引之發展

摘要

社區憂鬱老人的自殺防治是醫療照護關注的議題。本研究目的為透過憂鬱老人自殺評估的系統性文獻回顧，以專家焦點團體和醫護人員問卷調查建立指引，以提供在憂鬱老人照護中對自殺預防之依據。第一階段研究方法採系統性文獻回顧法，查詢2001年至2010年之文章，共篩選七篇高品質實證等級文獻，並發展出50項指引草案。第二階段使用專家焦點團體法，邀請12位專家進行訪談，進而修正指引草案。第三階段採取問卷調查法，調查相關醫護人員對指引的同意程度，共回收225份問卷，結果顯示專家與臨床照謢成員同意指引草案達99.03%。本研究結果能提供臨床專業人員以實證為基礎且高品質的照護指引，並有效評估憂鬱老人自殺的問題，以期降低老人自殺率並增加社區老人之生活品質。

It is a medical care issue to prevent suicide among depressive older adults in community. The purpose of this study is to develop a suicidal assessment and screening guideline for depressive older adults through systematic review, focus group and survey. The first stage used systematic review method. Seven high quality evidence-based articles from year 2005 to 2010 were selected to develop a guideline protocol with 50 items. The second stage used focus group method to revise the guideline protocol. Twelve professionals were invited in the focus group for interviewing. The third stage used questionnaire to survey the staffs related to this field for agreement of the guideline protocol, and 225 questionnaires were collected. The results showed that professionals and staffs agree the guideline protocol with 99.03% agreement. This study result can provide clinical workers an evidence-based and high quality care guideline to assess suicide of depressive older adults. It is expected to decrease suicide rate and increase life of quality among older adults in community.
