照顧一位前列腺癌患者接受高劑量率後荷式近接療程返室後的護理經驗照顧

摘要

本文主要是探討一位前列腺癌患者接受高劑量率後荷式近接療程返室後之護理照護過程。筆者於民國九十五年九月四日至八日三次療程中，透過觀察、訪談、傾聽及運用Gorden十一項健康功能性評估等方式，分析歸納出個案所呈現的護理問題為：潛在危險性傷害、舒適情況改變、知識缺失（對出院後的居家照護）。在照護過程中，協助調整正確姿勢，協助背部及雙下肢按摩及提供拔針與返家後的護理衛教，使個案在治療期間減少因置入治療針造成周圍組織的傷害，緩解治療中的不適並對返家後照護有較具體的瞭解。僅以此篇報告呈現護理人員在醫療機構照顧高劑量率後荷式近接治療所發揮的角色功能，並希望藉此護理經驗提供護理人員照護此類患者之參考，進而提昇護理照顧品質。

This article explores the nursing care process for a prostate cancer patient receiving high dose rate brachytherapy (HDR). The author used observation, interviewing, listening, physical assessment and the Gorden functional health assessment guide during the three therapy sessions from September 4 to 8, 2006. Three problems were identified: potential risk for injury, change of comfortable condition, and knowledge deficiency: homecare after discharge. During the treatment, the patient was assisted to adjust accurate position, received massage of the back and legs, and was provided nursing instruction for the after-needle removal care and discharge care. Therefore, the patient experienced minimized tissue injury because of medical treatment, release of physical discomfort and receiving comprehensive knowledge about homecare. This article provides insight into the role of nursing professionals for taking care of a patient with HDR. It is hoped that the nursing experience in this case can serve as a reference for nursing professionals in providing quality nursing care for this kind of patient.
