降低耳鼻喉腫瘤專科病房抗癌化學治療藥物外滲發生件數
Reducing the Event of Extravasation of Anticancer Chemotherapy Drugs in an Otolaryngology Ward

本單位於2018年發生抗癌化學治療藥物外滲事件，造成病人傷害及醫護人員壓力，引發專案動機。小組人員利用觀察、檢閱文件、訪談及問卷調查，確認造成藥物外滲導因為：一、作業標準規範不完整；二、護理人員植針技術不佳；三、護理人員對化學治療藥物預防外滲之認知不足；四、病人衛教指導內容未包含化學治療外滲預防與處理。經過專案小組與院方腫瘤護理組討論後提出解決辦法，包含：修改作業指導書及管理程序書、辦理教育課程、使用多媒體教材及化學治療警示掛牌等。2018年5月至10月單位未再發生藥物外滲事件，護理人員之認知正確率提升至97.3%、技術正確率100%，病人認知正確率100%，另將定期稽核護理人員植針技術列為單位常規作業。本專案能有效預防化學治療藥物外滲事件發生，提供病人安全的就醫環境及照護。

In 2018, one event of anti-cancer chemotherapeutic drug extravasation occurred in our unit that causing patient injury and huge stress to the medical team, which triggered the motivation of this project. The team employed investigated the causes of drug extravasation by observation, documented review, interviews, and questionnaires. The results indicated that the causes of drug extravasation included: 1) incomplete standard operating procedure; 2) incorrect nursing needle-insertion technique; 3) insufficient awareness of extravasation in staff; and 4) insufficient awareness of extravasation in patients. Our goal was to eliminate such extravasation events. The solutions included: modifying the operating standards, holding educational courses, utilizing multimedia materials, and labeling chemotherapeutic warnings. No further drug extravasation event occurred from May to October in 2018.The correct rate of needle-insertion was increased to 100%, and the awareness in staff and patients increased to 81.3% and 100% respectively. We now supervise the skill of the medical staff in needle-insertion regularly as a routine operation. This project can effectively prevent the occurrence of extravasation of chemotherapeutic drugs.
