本文描述一位口腔癌病人,因營養攝取不足,入院接受經皮內視鏡胃造口術之護理經驗。照護期間自2019年6月4日至6月25日,追蹤至2019年7月2日,運用整體性護理評估模式,以觀察、會談、身體評估、查閱病歷及電訪等方式收集資料,分析個案有營養少於身體需求、組織完整性受損、身體心像紊亂等健康問題。提供個別性護理計畫,以多媒體影片及手冊,教導均衡飲食的重要性,增加其營養攝取;以換藥口訣及自製換藥包,教導傷口照護技巧,增強其組織完整性;傾聽個案表達心裡感受及修飾身體外觀,改善其身體心像受損,期盼藉此照護經驗分享,可供護理人員日後照顧此類病人之參考。
This paper describes a nursing experience of a patient with oral cancer after receiving percutaneous endoscopic gastrostomy due to insufficient nutritional intake. The nursing period was from June 4 to 25, 2019 and the patient was followed up to July 2, 2019. Data was collected by observation, interview, physical assessment, medical records review and telephone interview. A holistic nursing assessment model was used to assess nursing problems including decreased nutrition intake, impaired skin integrity, and disturbance of body image, then an individual care plan was provided. Firstly, videos and manuals educated the patient in the importance of increased nutrition and a balanced diet; secondly, wound care skills and dressing tips were taught to the patient to enhance skin integrity; and finally, by listening to his feelings and modifying his physical appearance, his impaired mental image was improved. This experience provides a good reference for practical nursing.
