居家靜脈營養(Home parenteral nutrition, HPN)提供腸道手術後病人居家營養支持,因此護理人員具備正確居家靜脈營養相關知識及提供護理衛教是必要的知能。查核本單位二年期護理師執行居家靜脈營養認知正確率僅48.6%及技能正確率亦僅51.4%,原因包括缺乏護理照護標準及衛教輔助工具、缺乏教育訓練、教學方式不一致、缺乏實際操作經驗及稽核機制等。因此擬定對策,包括制定居家靜脈營養病人之照護標準,並拍攝居家靜脈營養照護過程影片、舉辦實體教育訓練課程,及執行客觀結構式臨床測試(OSCE)等,專案執行後二年期護理師執行居家靜脈營養認知正確率提升為94.2%,技能正確率提升為94.2%,顯示運用多元教學模式能提高學員臨床照護的指導正確率。
Home parenteral nutrition (HPN) provides home nutrition support for patients after enteral surgery. Therefore, specialized nurses should have the associated knowledge and the ability to educate patients and family to perform HPN procedures. Previously, the accuracy rate of the nurse post-graduate year (NPGY) in our institute in knowledge of HPN was poor at 48.6%, and the accuracy rate of HPN skills was only 51.4%. The causes include lack of nursing care standardization and instructional equipment, education training, inconsistent teaching methods, practical experience and audit mechanisms. Therefore, the proposed resolutions include conducting an educational training program, implementing objective structured clinical testing (OSCE), recording videos of the HPN care procedure, and setting up care standards for HPN patients. The implementation of the project improved the NPGY in our institute in both accuracy rate of knowledge and skills up to 94.2%, demonstrating that a diversified teaching model is an excellent method to improve clinical care tasks for students.
