照顧一位裝置永久性心臟節律器病患的護理經驗
Nursing Experience in a Permanent Pacemaker Implantation Patient
摘要
本文係呈現一位86歲老年患者在接受永久性心臟節律器植入術前後，所面對的生理、心理問題，於護理過程中運用Gordon十一項功能性健康型態做一整體性評估，結果發現個案有心肺組織灌流改變、高危險性傷害／跌倒、焦慮、高危險性感染、睡眠型態紊亂及健康尋求行為等護理問題；護理人員在照顧此個案期間除監測生理變化外，並提供足夠的心理支持及護理指導，以協助個案及家屬渡過手術過程及執行返家後自我照顧的準備。

This article describes an 86 year-old man who had accepted permanent pacemaker implantation, with physical and mental problems. The nursing period was between Feb. 11 and Feb. 20, 2001. The Gordon 11-item health assessment was used to identify problems in the nursing process. The results of nursing diagnosis were as follows: altered cardio-pulmonary tissue perfusion, high risk for fall-related injuries, anxiety, high risk for infection, sleep pattern disturbance and need for health-seeking behaviors. Thus, the nurse not only monitored physical conditions, but also provided mental support and nursing teaching during hospitalization. The client and family were able to deal with the surgical procedure, and were better prepared for self-care after discharge.
