海洛因成癮者接受替代治療出席率低改善方案

A Project for Improving Low Attendance Rates of the Replacement Therapy of Heroin Addicts

摘要


背景:海洛因成癮會衍生複雜的個人、家庭、治安及公共衛生問題,通常使用替代治療來達成長期治療目的,然而最大困難是治療後有很高的治療流失率與復發率。目的:提升海洛因成癮者接受替代治療出席率為90%,使藥癮者獲得最佳治療效果,改善治療流失率與復發率。方法:修訂電話追蹤作業規範、制定出席率管理作業及可近性給藥時段並積極推動丁基原啡因藥物。結果:出席率由88.1%提升至91.67%,持續追蹤可再提升到93.23%。結論/實務應用:本專案藉由修訂電話追蹤流程、增強個案的正確認知、制定出席率管理作業及提升服藥可近性,來促進藥癮者接受替代治療意願,達到最佳治療效果。

Background: Heroin addiction is associated with various individual, social and public health problems. Replacement therapy has been implemented to achieve long-term abstinence efficacy. However, the main obstacles for successful replacement therapy are low retention rate and high relapse rate. Purpose: Our aim is to increase the attendance rate of replacement therapy to 90% and above in order to attain optimal treatment efficacy, higher retention rate and lower relapse rate. Methods: We achieved this goal by revising the protocol of telephone follow-up, by implementing attendance monitoring system and by prolonging the service hours, In addition, alternative treatment with buprenorphine was encouraged. Result: The attendance rate increased from 88.1% to 91.67% within ten months, followed by increasing to 93.23% in the next four months. Conclusion / Implications for practice: Our project suggested using telephone follow-up actively, improving participants' cognition, implementing attendance monitoring system and increasing accessibility to replacement therapy can enhance the abstinence motivation of individuals with heroin addiction to obtain optimal treatment effect.
