醫療不良事件指醫療處置造成病人傷害、失能或延長住院等。本單位於2017年6月發生護理人員操作遠紅外線儀不慎致病人右膝誤觸儀器造成二度燙傷需傷口清創之事件。故成立專案小組,設立認知調查表及執行遠紅外線儀查檢表,經查檢發現護理人員對遠紅外線儀認知正確率62.5%、操作正確率67.6%,經問題確立為缺乏照射標準作業流程、教育訓練、標準化醫囑及輔助工具等。擬定對策為:制定遠紅外線儀照射標準作業流程、教育訓練、設計照射安全姿勢擺位圖、設置標準化醫囑及護理記錄及製作遠紅外線儀操作教學電子書。改善後護理人員認知正確率提升為98.7%、操作正確率提升為97.5%,顯示措施介入後確實能有效降低醫療不良事件並提升病人安全品質。
Medical adverse events (AEs) indicate that medical intervention or prolonged hospitalization causes harm and disability to patients. In June 2017, a nurse in the unit of this study performed a far-infrared (FIR) therapy causing an accidental second-degree burn to the patient's right knee that needs wound debridement. Thus, a special group was set up to roll out the Perception Survey Questionnaire and the FIR checklist. The result showed that nurses have 62.5% and 67.6% correct recognition and operating rates, respectively. It was confirmed that the problem of accidentally causing the burn to the patient was due to the lack of knowledge with the familiar operating procedures and incomplete education and training (e.g., setting up standardized medical orders and auxiliary tools). Formulating standard operating procedures for FIR, completing education and training, designing irradiation safety posture maps, setting standardized medical orders, nursing records, and making use of far-infrared instrument teaching e-books are the corresponding countermeasures used. After the procedures, the post-test showed that the nurses have 98.7% and 97.5% correct recognition and operating rate, respectively. Consequently, the above interventions could effectively reduce medical AEs and improve the quality and safety of nursing care.
