Bundle branch block is a delay or blockage of the electrical impulse pathway of the heart. It is not an uncommon finding during aircrews' annual physical examination. In most people, bundle branch block doesn't cause symptoms. However, in rare situations, fainting (syncope) or feeling as if you're going to faint (presyncope) may develop. Risk factors for bundle branch block include increasing age and underlying health problems such as high blood pressure or heart disease. A thorough evaluation is needed with new onset bundle branch block or those with symptoms. In most cases, right bundle branch block without symptoms need no treatment. However, left side bundle branch block which is often due to underlying organic cardiac diseases may need treatment. The flight surgeons should understand the aeromedical significance of bundle branch block and handle each case deliberately.
