叢發性頭痛之中醫治療病例報告

A Case Report of Cluster Headache with Traditional Chinese Medicine

摘要

叢發性頭痛是一種好發於青壯年男性的單側劇烈頭痛，一般的止痛劑，例如：Aspirin、Acetaminophen、NSAIDs治療效果不好。本醫案是一位58歲女性於去年12月底寒冬開始遭受連續10天的右側頭部劇烈抽痛，每當症狀發作時有頭痛欲求死的感覺，幾乎每晚皆需到西醫急診部求診，甚至有時一天要就診2-3次，對患者造成很嚴重的困擾。經友人介紹來嘉基中醫部就診，經診斷後給于黃耆桂枝五物湯加減，服用第一帖湯藥後，當晚頭部抽痛感由原本的10分減為2分，連服三周的藥物後，頭部抽痛已完全消除。之後追蹤2個月，也都未再發生頭部抽痛等不舒服的症狀，因此提出個案探討臨床思路與中醫同好分享。

Cluster Headache is an unilateral severe headache, which is frequently occurred in young men. General analgesics, such as Aspirin, Acetaminophen and other non-steroidal anti-inflammatory drugs (NSAIDs) are not effective medicines against the pain. A 58-year-old female was referred to the clinic at the end of December last year, presenting with severe throbbing pain on the right side of her head for 10 consecutive days. She can't endure the pain and go to the emergency room almost every night. The patient was prescribed huang gi gui zhi wu wu tang. After taking one quote drug, the patient felt that the pain scale of headache was significantly decresed from 10 to 2. Finally, the patient's headache was gone after 3 weeks' treatment, and has not been recurrent for two months.
