腫瘤增生造影劑的發展與應用

The Development and Application of Tumor Proliferation Tracers in PET

摘要


癌症被公認爲國人健康的頭號敵人,國內外專家學者對於癌症的研究已投下不少心力,遺憾的是,癌症死亡率仍高居不下。早期發現且早期治療是現今對付癌症的不二方針,因此,如何早期發現癌症即爲目前努力的目標。在腫瘤發生早期時,不受控制的快速細胞分裂是癌細胞的特性之一,此外,許多治療癌症的方法都以抑制細胞分裂爲手段,腫瘤生長速率自然是評估治療成效最準確的方式。傳統評估細胞或組織生長速率都需以侵入式切片做免疫分析(如:Ki-67 assay),若能以非侵入式造影得知此資訊,非但增加了受檢者之方便性及降低其不適感外,亦對癌症診斷、分期或是治療評估多所助益,或可達到早期發現的目的。核醫造影之影像解析度雖不若CT或MRI,卻可提供不同於解剖影像的功能性影像,因此,選擇一個適合的腫瘤增生速率示蹤劑配合核醫造影實爲重要的關鍵。本篇文章將介紹各種腫瘤增生造影劑的發展,並探討其優缺點及未來進一步研究的方向。

Cancer has been the leading cause of death in Taiwan till now. Researchers around the world have exerted themselves on promoting tumor detection efficiency and cancer therapeutic efficacy. Early detection and early treatment is the best strategy for cancer therapy. Uncontrolled cell division is the defining characteristic of cancer, especially in early stage. Therefore, the goal of early detection could be achieved by imaging the proliferation rate of cells. In addition, many anticancer therapies are aimed directly at inhibiting cell division in tumors. Tumor growth rate is considered an usefully prognostic indicator of tumor aggressiveness and tumor response to therapy. The ability to image cell growth rates noninvasively throughout the body could be of great value in the diagnosis, staging and treatment of malignancies. In contrast to CT or MRI, nuclear medicine imaging no matter gamma imaging or PET imaging, could provide functional information of tumor biology. Researchers have been devoting efforts to develop PET proliferation markers specific for imaging of tumors. In this review, the development of each kind of proliferation tracer would be introduced and the comparison of these tracers was also conducted on.
